Lakeland Regional & HCA Clinical Training Engine

Co-written by David Dombrowski & John Jensen. Originally built in Classic ASP/VBScript connected to MSSQL Server and Microsoft Access databases to process Lakeland Regional Medical Center's 10,000-tape instructional video library. Resurrected here in native PHP 8 with zero-cost embedded database engine.

Vintage 1994 Architecture: Windows NT / IIS 4, Active Server Pages (VBScript), ADODB.Connection, Microsoft SQL Server 6.5 & Access (.mdb) files, Netscape Navigator / IE3.
Modern Resurrected Stack (2026): PHP 8.2+, Free Embedded Relational Engine, HTML5 Video Canvas, Cryptographic SHA-256 Certificate Verification, Fully Responsive.

Sterile Field & Aseptic Operating Room Protocols

Comprehensive standard operating procedures for maintaining surgical sterile boundaries, glove technique, drape placement, and airborne pathogen barrier controls. Originally digitized from LRMC 10,000 video library.

Interactive Training Curriculum & Step Modules

Step 1: Pre-Operative Scrubbing & Barrier Gowning

All surgical staff must perform a 5-minute chlorhexidine gluconate scrub. Hands must remain elevated above elbows at all times during rinsing. Do not allow sterile gowns to touch non-sterile surfaces.

💡 Remedial Rule: Remedial Review: Remember that hands must always stay higher than elbows to prevent water runoff down to clean forearms.

Step 2: Establishing the Sterile Field & Mayo Stand Draping

The sterile field must be prepared as close as possible to scheduled incision time. Drapes must be shaken gently without billowing. Sterile boundaries are strictly waist-high to table level.

💡 Remedial Rule: Remedial Review: Anything below table level or draped off the edge is considered contaminated.

Step 3: Intraoperative Breach Mitigation & Re-Gloving

If a tear occurs in a surgical glove or fluid strikes a drape, the entire instrument or glove must be replaced immediately. Circulating nurses must assist without crossing the sterile threshold.

💡 Remedial Rule: Remedial Review: Never reach across the sterile zone. Step back and call the circulating nurse.

Lakeland Regional Video Digitization Stream

In 1994, David Dombrowski converted 10,000 analogue magnetic tapes into interactive digital modules with synchronized timecodes.

Clinical Competency & CEU Certification Exam

1. When scrubbing for the OR, what is the mandatory position of the hands relative to the elbows during rinsing?
2. What portion of a draped surgical table is classified as sterile?
3. If a sterile drape becomes damp through capillary strike-through during surgery, what is the required clinical action?
COURSE CODE: CARD-204 • 3 CEU HOURS • Critical Care / Telemetry

Continuous Telemetry & Lethal EKG Arrhythmia Identification

Waveform analysis of V-Tach, V-Fib, Asystole, Third-Degree Heart Block, and rapid intervention protocols for cardiac care units.
